Baby Phillip Guy Cook passed away Monday, August 8, 2016. Now an angel in heaven, Phillip leaves behind his mother and father, Cristine and Phillip Cook; his sister, Caitlin; his grandmothers, Marianne and Mary; and his aunts and uncles, Phil and Katie, Sara and Rich, Darlene and Jeff, Mike, Gabe and Jerrianne.
